Bed Leveling failed

I've already swapped the extruder cable

Since the Z-axis looks to be working when initially homing, there is a small chance this might be a software-related issue since it's failing when attempting to probe but works when homing.
*could also be related to a bad:
- Z-axis stepper motor
- Stepper driver
- Probe
- Z-axis stepper motor cable
